Levels of Care
The Four Types of Hospice Care We Offer in Troy
Because Troy families have different lifestyles, needs, and levels of care required, Choice Hospice provides four distinct levels of hospice services. We work with each patient and family to match the right level of care to where they are today and adjust as their needs evolve.
Routine Home Care is the most common level of hospice care provided to patients at their place of residence, whether that is their own home, a nursing facility, or an assisted living community right here in Troy.
Under this level of care, our skilled local care team visits regularly to manage symptoms, coordinate medications, and provide the full range of hospice services. Patients must meet eligibility criteria per federal and state guidelines.

Choice Hospice arranges short-term inpatient hospice stays of up to five consecutive days in a Medicare-approved facility, such as a hospice inpatient facility, hospital, or nursing home. Respite Care is available for up to five days out of every 30 days of hospice service and our team handles all the arrangements for your family.

Continuous Home Care is provided during periods of medical crisis, when a patient requires intensive, round-the-clock 24-hour home care at home or in a residential facility.
This level of care is designated for patients experiencing acute symptoms as defined by the Hospice Rules of Health Care Financing Administration (HFCA). During a crisis period, Choice Hospice arranges continuous hospice nursing and aide services. A minimum of 8 hours of continuous care is required and if continuous care extends beyond 18 to 24 hours, a transition to inpatient care may be indicated.

General Inpatient Care is reserved for patients experiencing acute symptoms that cannot be effectively managed at home or in an outpatient setting. When a patient’s condition requires a higher level of clinical oversight, Choice Hospice arranges short-term inpatient care in an approved facility staffed by a hospice nurse and registered nurse (RN) on-site 24 hours a day, seven days a week.
This ensures that even the most medically complex situations are met with the expertise and attentive compassion that every Troy patient and family deserves.
